Private Health Insurance Fund
4livinghealthy dietetics practice is registered with all private health funds. Individuals with private health insurance (extras) may be entitled to rebates for Accredited Practicing Dietitian (APD) services. Please check eligibility with your fund. Consultation item codes are 500 (initial consultation), 600 (review consultation). Receipts for services will be provided for claims reimbursement from your health fund.
Medicare
To see one of our dietitians under Medicare, you will need to be referred by your GP. Once your book an appointment with your GP and talk about your needs, your GP will assess whether you are eligible for Dietitian rebates under a GP Management Plan and Team Care Arrangements. Your GP will complete a referral form for you which needs to be emailed to us at office@4livinghealthy.com or brought with you during your appointment.
The number of sessions that you are eligible for depends on your individual circumstances, though generally there are up to 5 sessions available that you can claim a rebate of approximately $60 per session. Our dietitians can help manage your chronic health conditions such arthritis, diabetes, heart disease and weight related conditions.
You will be required to pay for the full consultation fee, then we will help you claim the Medicare rebate on the spot with your Medicare card and the funds are generally back in your account within 24 hours. Please be aware that we are not a bulk billing clinic.
We have a 24-hour cancellation policy. Please respect that all cancellations and rescheduled appointments should be made with at least 24 hour’s notice via a text (0482664749) or email office@4livinghealthy.com
If an appointment is cancelled or changed within 24 hours of the original appointment, a $75 fee may be charged.
